AUTOMATIC ELECTRONIC TRANSFER OF PATIENT RECORDS GP2GP TRANSFER
This facility allows patients electronic records to be safely and quickly transferred from one practice to the next as a patient re-registers on a permanent basis with a new surgery.
The majority of our patients records are already transferred electronically to or from us as patients register and de-register where the other surgery is on the same clinical system as us
Access to Your Health Records
Under the provisions of the Data Protection Act, patients have a right to see their health records. This includes information on manual records as well as computer system.
In order to access this data, it is necessary to make a written request to the practice. There is a charge if you require a copy of the information.
Our staff are able to help and advise patients who require access to their information.
Fair Processing Notice
Our practice has been asked to take part in a pilot with NICE.
The aim of the pilot is to test potential new general practice quality indicators in a representative group of general practices in England.
Piloting is designed to improve the quality of service you receive from your GP, to develop good practices which can be used nationwide.

Statement of Intent
New contractual requirements came into force from 1 April 2014 requiring that GP Practices should make available a statement of intent in relation to the following IT developments :
1. Summary Care Record (SCR)
2. GP to GP Record Transfers
3. Patient Online Access to Their GP Record
4. Data for commissioning and other secondary care purposes
The same contractual obligations require that we have a statement of intent regarding these developments in place and publicised by 30 September 2014.

Our responsibilities to you are to maintain the contents of your records confidentially and to allow you to see your records as required, under the Subject Access Request regulations.
Medical records in this practice contain statements of opinion about the likely diagnosis accounting for your problem at a particular time.
This diagnosis is entered in order that we can monitor the basis of our actions such as prescribing interventions, etc.
Since it is an opinion expressed at the time it is not unusual for the final diagnosis in any episode to differ from it.
Often when illnesses first present to a doctor, the range in severity of symptoms does not allow more than a tentative diagnosis.
